Wallet Card

It all started when...

Our youth outreach workgroup expressed concern that teens in our county, had little knowledge of the resources they can access, especially confidential access, things like birth control or STI testing and treatment.

And so we created a handy tri-fold card that can be carried in a wallet, with important local contacts and mobile phone apps.

Family PACT

What is it?

Family PACT is a California health program that provides FREE family planning and related services. More information here www.familypact.org

What does it cover?

FDA approved contraceptive methods and supplies, family planning counseling and education, sexually transmitted infection (STIs) testing & treatment, HIV screening, cervical cancer screening, male and female permanent contraception, and limited infertility services.

who is eligible?

Anyone who needs family planning access, care and treatment, and is low income and/or in need of confidential services.

HOw do I get it?

Call and make a "Family PACT appointment" at one of the local providers:

Lake Tahoe area:

They will help you complete the paperwork, sign you up, and give you a HAP card, like this:

Do I need the consent of my parents or others?  Family planning services do not require the consent of anyone other than the person who is to receive services.  If you are 17 years of age or younger, you are considered a minor and may apply for family planning services on the basis of your need for these services, without parental consent. 

What does it cost?  It's FREE.  As a Family PACT client, you are never to be billed for Family PACT benefits.  However, always verify your eligibility before a procedure to ensure you still qualify for Family PACT and your HAP card is current.

How long does it last? One year, renewal is easy.

What if I travel out of state? The program does not provide prescriptions or refills to Family PACT clients out of the state, even if it is temporary. The program would also not be able to reimburse you for prescriptions provided by an out of state provider or pharmacist.

What other healthcare services does it provide?  Family PACT is not a primary care program.

Keep it confidential

You are automatically eligible for confidential Family PACT benefits, even if you have other healthcare coverage but are concerned that your parent, spouse or partner may learn about your family planning appointment.  This applies to all clients regardless of age or marital status.
